[A] Budget: Looking for Proposals

[B] Scope of Service:

(1) Vendor needs to provide jail-based cognitive behavioral services to the government authority located in Clinton Township, MI.
- To provide SUD assessments, case management, and cognitive behavioral services for those currently supervised on probation or incarcerated
- Case management services continue if the individual is enrolled in continued programming. Jail-based cognitive behavioral programming occurs in several housing areas utilizing evidence-based criteria and approved for both male and females
- Have the necessary systems in the areas of administration and clerical support for the program. This includes the necessary computer equipment, compatible software to submit data and billing
- Provide information that all employees used for this program are screened. The employees used for this program should not have and pending criminal prosecution, should not be on probation/parole supervision or have a prior felony or misdemeanor conviction unless approved
- The Provider shall submit progress report forms in the format supplied by Community Corrections for each client detailing services provided.
- The Provider shall submit a vendor invoice by calendar month by the 10th day of each month for services provided the previous month. Payment is in monthly intervals with an invoice for services rendered provided by the selected provider that includes dates of service, service provided and names of participants etc.
(2) All the questions must be submitted no later than May 26, 2022.
