[*] Scope of Service:

(1) Vendor needs to provide professional occupational medicine services to the government authority located in Lockport, NY.
- professional services from an occupational medicine provider and its licensed physician(s) that possess all required certifications and qualifications for providing professional occupational medicine services. The County is a municipal employer employing approximately 1450 employees and will require an estimated 250 to 350 employee testings per year
- The services expected to be needed are primarily but not limited to the following:
- Pre-employment physicals and drug screening,
- Reasonable suspicion testing for drugs and alcohol,
- A variety of annual physicals such as those employed in public health with direct patient/clinic care,
- Random drug screen,
- DOT physicals,
- Referral for medical treatment and evaluation due to reported sickness or injury while on the job,
- Non-DOT physicals and drug screenings.
(2) All the questions must be submitted no later than November 22, 2023.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
